BrainAche Productions Actor Casting Form

Greetings Actors! BrainAche Productions has placed many casting calls throughout the years, but inevitably, most of the responses I receive are missing one or more bits of needed information. To combat this problem, I am implementing this Casting Call Form, which all actors are required to fill out if they want to be considered for a part in one of my films. I understand that some of the information requested on this form will also be included on your resume, but please complete the form anyway.

When you reach the bottom of the form, you will have an opportunity to add a personal note, attach your resume, plus send one or two photos.
